Transforming Care Delivery: Introduction

The goal of transforming healthcare is to help providers ensure that the care they deliver meets the six Institute of Medicine (IOM) aims- it is safe, effective, efficient, equitable, timely and patient-centered. The Transforming Care Delivery module of the Toolkit addresses methods for transforming the way that care is delivered in the clinician’s office, and redesign efforts that further engage physicians in health information technology (HIT) and health information exchange (HIE). 

Redesign of care delivery will be extremely difficult without significant investment in information technology (IT) infrastructure to improve access to clinical information; support clinical decision making; support coordination of care across patient conditions, services and settings over time; and facilitate performance measurements for quality improvement and accountability as well as changes in reimbursement policy. At the heart of the redesign process are the clinicians delivering care and the patients who receive health care.

This module is not meant to repeat the work of other organizations (IOM, ACP, AAFP and others), but to leverage and synthesize the common themes emerging in health care transformation and the significant role of HIT and HIE.  It is designed as a reference for practicing clinicians and communities to highlight methods of care redesign and to provide a practical road map for those clinicians on the practice transformation journey.
