The introduction of health IT and health information exchange holds great promise for improving care coordination. The National Quality Forum has defined care coordination as a "function that helps ensure that the patient's needs and preferences for health services and information sharing across people, functions, and sites are met over time." The eHealth Initiative (eHI) is developing a framework to assess the systems needed to help navigate across the care continuum.
